    As an authorized representative of the aforementioned insurance policy, I hereby acknowledge sending this request to add a driver does not necessarily and automatically include the driver in question to be included on my insurance policy. I also understand it takes 24-48 hours from the time of this request to receive a written and formal approval/denial from my insurance company regarding the status of my request.
    Acknowledgement of this form will be your copy of our change request sent to the insurance company. If you do not receive an acknowledgement within five days, please notify us. No coverage changes will be in effect until you receive confirmation from our office.

    ​All endorsement requests received after 4:00 pm will be processed the next business day after 9:30 am.
